Transportation Service Notice Driv is an independent contractor and is not an employee of Dr. Wendy’s office. His transportation services are made available as a courtesy for patients who do not have access to their own transportation. This service is provided at no cost to the patient. Transportation is limited to direct travel only. Dr. Wendy’s office does not cover detours, additional stops, or alternate destinations. The location scheduled for pick-up will be the same designated drop-off location, unless a change is approved in advance through the schedule coordinator. Please be aware that although the office assists with scheduling, this is a private transportation service, and the office assumes no responsibility or liability for any accidents, injuries, delays, or incidents that may occur during transport.
